I covered this in the last part of my reply:

Another possible issue is windows lowering down the volume for out of focus app automatically, but there's a setting in Windows to prevent doing this.

As mentioned my lack of GSX sound is intermittent.  Today it did not work.  Log file attached in case this can shed any light.
