Title: No audio from pushback

All working well except that the sequence when the ground crew contacts the captain prior to pushback, tells him to release brakes, start, etc. is missing.  All other audio is fine.

With "all other audio is fine", you mean the other audio in the sim, or the other GSX audio, so you are missing JUST the Pushback sounds ?

Audio from both FSX and GSX is fine except no audio during GSX pushback.

Then be sure you have installed GSX using the latest GSX full installer, maybe you missed some major update in the past, so you are missing some files.
